  2. Driver/Vehicle Information: Obtain name and driver license identification number of all drivers involved in the accident, as well as the license plate number and insurance information for all vehicles involved in the accident;
  3. Witness Information: Obtain name, address and telephone number of all witnesses to the accident, if any;
  4. Photographs: Obtain photographs of accident scene and damage sustained to all vehicles involved in the accident; and,
